Preventing fasting

How long is safe to fast?

In short

Age-specific safe fasting limits are documented from the Derks 2007 study: 8 hours (6 months–1 year), 10 hours (second year of life), and 12 hours (after the second year). These apply to well children; fasting must be minimized during illness.

A key reference for fasting duration is Derks et al. (2007), which examined fasting tolerance in 35 fasting tests in Dutch MCAD-deficient patients. Age-specific recommended maximum fasting limits are:

Important findings from Derks 2007:

  • In 6 out of 35 fasting tests, symptoms were reported before hypoglycemia was observed — symptoms can precede biochemical hypoglycemia.

  • No conclusions can be drawn on fasting duration during intercurrent illness, especially with fever — the limits above apply to well children only.

Risk stratification by residual enzyme activity (Touw 2012):

  • Residual activity <10%: same risk as the classical genotype — full fasting precautions apply.
  • Residual activity >10% (variant genotypes): arguments exist for abandoning the general advice to avoid prolonged fasting.
  • The effect of intercurrent illness on residual enzyme activity remains uncertain — fasting precautions should still be maintained during illness regardless of genotype.

Meals and macronutrients

In short

A normal, balanced diet with regular meals is the aim. Carbohydrates are the safe energy source, fats are not generally restricted, and no special protein restriction is needed.

Carbohydrates

Carbohydrates are the primary safe energy source for individuals with MCADD. Regular carbohydrate intake helps maintain blood glucose and prevents reliance on fatty acid oxidation.

Fats

Dietary fat is not generally restricted in MCADD (unlike some other metabolic conditions). However, the specific type of fat may matter (see the MCT section below). GeneReviews does not recommend a strict low-fat diet for MCADD.

Proteins

No specific protein restriction is recommended for MCADD. Normal dietary protein intake is appropriate.

Managing illness

Fever

Fever increases metabolic demand. Adequate caloric and fluid intake must be maintained. Antipyretics may be used as directed by a physician.

Gastroenteritis (vomiting / diarrhea)

Vomiting and diarrhea are particularly dangerous because they prevent caloric intake and cause dehydration. If oral intake cannot be maintained, intravenous glucose may be required. This is a potential emergency situation.

When to seek medical attention

What are MCTs?

Medium-chain triglycerides (MCTs) are fats composed of medium-chain fatty acids (C6-C12). MCTs are found in coconut oil, palm kernel oil, and some dietary supplements.

Historical and current recommendations

Historically, there has been concern that MCTs may be dangerous for individuals with MCADD because they are metabolized through the same pathway that is deficient (medium-chain fatty acid β-oxidation).

Orphanet recommendation on MCT

Orphanet (ORPHA:42), an authoritative Tier A source, explicitly states: "Les triglycérides à chaîne moyenne doivent aussi être évités" (medium-chain triglycerides should also be avoided). This recommendation comes from the Orphanet clinical summary for MCADD (last updated February 2014).

Other dietary considerations from Orphanet:

  • Artificial sweeteners should be avoided.
  • Low-dose carnitine in patients with low carnitine levels is controversial (no consensus).
  • Symptomatic patients: simple carbohydrates administered orally (glucose tablets) or intravenously until glycemia is maintained above 5 mmol/L.
  • No other dietary restriction is required beyond MCT avoidance and fasting prevention.
